To search for a provider, click here to login to the UHC member portal.You can also search for a provider by navigating to the "Find a Provider" link. After entering your location, select the Choice Plus option.
Your prescription plan is managed by UnitedHealthcare and uses Optum Rx. Click this link and navigate to "Find a Pharmacy" to locate a local in-network pharmacy.
