The Valenz Surgical & Imaging Program (formerly the KISx Card Program) allows you to pay $0 out of pocket for many common procedures. 

 

For example, instead of paying your normal copay or deductible for a knee replacement, you could now receive the procedure at no cost. If you are enrolled in our HDHP 2750 medical plan, this service will be $0 after reimbursement.​

How it Works​

You can follow the steps below on your own, or you may contact your Lacher Healthcare Advocate, who can help guide you through the process and manage the coordination for you.

1. Contact Valenz - When you need a surgery or imaging procedure, call 1-877-438-5479 before scheduling with an outside provider.
    

2. Referral Received - Valenz receives your provider’s order and reviews it for eligibility.
    

3. Confirm Eligibility - Valenz verifies your benefits and confirms procedure details with your provider.
    

4. Scheduling Support - Valenz recommends nearby facilities convenient to your home or work and assists with scheduling.
    

5. Receive Your Voucher - Valenz emails you a voucher to present at the facility instead of your insurance card on the day of your procedure.

Who is Eligible?

This benefit is available to employees and their covered dependents who need major imaging or non-emergent surgery.

• Must be 18 years or older for surgery benefits

• Approximately 10 years or older for imaging benefits
